Access to ear, nose, and throat (ENT) and hearing care is often limited outside urban centres, yet the evidence on who provides this care in rural settings, and how that workforce is recruited, retained, and supported, has not been systematically mapped.\n\nThe review addresses three questions: (1) How are otolaryngologists and audiologists distributed geographically, and how does rural supply compare with urban? (2) What factors influence rural practice choice among ENT surgeons and audiologists, and what recruitment and retention strategies have been described? (3) What role do non-ENT clinicians, such as general surgeons, family physicians, and nurse practitioners, play in providing ENT care and procedures in rural areas?\n\nThe review follows the JBI methodology for scoping reviews and will be reported according to the PRISMA-ScR checklist. Eligibility is defined using the Population-Concept-Context framework. MEDLINE (PubMed), Embase (Ovid), and CINAHL (EBSCOhost) were searched in September 2026 for English-language studies published from 2000 onward. Screening had not begun at the time of registration. Two reviewers will independently screen titles, abstracts, and full texts, with disagreements resolved by a third reviewer. A Medicina do Estilo de Vida organiza esses comportamentos em seis pilares: alimentação, atividade física, sono restaurador, manejo do estresse, redução ou eliminação do uso de substâncias tóxicas (como tabaco e álcool) e manutenção de conexões sociais positivas. Além de sua relação com o desenvolvimento de doenças crônicas e multimorbidade, esses comportamentos têm sido associados a processos biológicos subjacentes ao envelhecimento. Entre esses processos estão a instabilidade genômica, as alterações epigenéticas, a disfunção mitocondrial, a senescência celular, o estresse oxidativo, a inflamação crônica e as alterações na dinâmica telomérica. Não existe um único biomarcador capaz de representar toda a complexidade do envelhecimento biológico. Por isso, as evidências sobre sua relação com o estilo de vida encontram-se dispersas entre diferentes pilares comportamentais, populações e indicadores. Diante da ausência de uma síntese abrangente que integre os pilares do estilo de vida aos indicadores de envelhecimento celular, propõe-se esta revisão de escopo.\n\nO objetivo é mapear as evidências disponíveis sobre a relação entre os pilares do estilo de vida e o envelhecimento celular em adultos e idosos. A pergunta norteadora, formulada a partir do acrônimo PCC, é: \"Quais são as evidências disponíveis sobre a relação entre os pilares do estilo de vida e o envelhecimento celular em adultos e idosos?\". A população (P) compreende adultos e idosos. O conceito (C) corresponde aos pilares do estilo de vida: alimentação, atividade física, sono, uso de tabaco e álcool, gerenciamento do estresse e relações sociais. O contexto (C) é o envelhecimento celular, entendido como o conjunto de alterações moleculares e funcionais associadas ao processo de envelhecimento.\n\nA revisão segue as recomendações metodológicas do Joanna Briggs Institute (JBI) para revisões de escopo e o checklist PRISMA-ScR. As buscas serão realizadas nas bases PubMed, Scopus e LILACS, e contemplarão estudos publicados os anos de 2016 a 2026. Os descritores controlados (MeSH e DeCS) serão combinados a palavras-chave livres e sinônimos por meio dos operadores booleanos OR e AND. Serão incluídos estudos originais com adultos e idosos, publicados em português ou inglês, que investiguem a relação entre fatores do estilo de vida e ao menos um biomarcador de envelhecimento celular. Entre esses biomarcadores estão o comprimento dos telômeros, a atividade da telomerase, os marcadores de senescência celular, a idade epigenética, o dano ao DNA, o estresse oxidativo, a inflamação e a função mitocondrial. Serão excluídos estudos com animais ou modelos in vitro, revisões, editoriais, cartas, resumos de congresso, preprints e estudos sem texto completo disponível. Os dados serão extraídos por meio de um instrumento padronizado, estruturado segundo o PCC. Esse instrumento contempla a identificação e o desenho dos estudos, as características da população, os pilares investigados e seus métodos de avaliação, os indicadores de envelhecimento celular e as técnicas de mensuração, a direção das associações observadas, as medidas de efeito e os fatores modificadores.\n\nComo resultados, espera-se identificar e caracterizar o corpo de evidências sobre o tema. O mapeamento deverá mostrar quais pilares do estilo de vida foram mais e menos investigados, quais biomarcadores de envelhecimento celular são mais utilizados e quais métodos são empregados na avaliação do estilo de vida e do envelhecimento celular. Também deverá indicar a direção predominante das associações relatadas. A revisão pretende ainda evidenciar lacunas de conhecimento, como pilares pouco explorados (por exemplo, relações sociais e manejo do estresse), a escassez de avaliações integradas de múltiplos comportamentos e a heterogeneidade metodológica entre os estudos. Os resultados serão apresentados em fluxograma PRISMA-ScR, tabelas e síntese narrativa. A likelihood ratio above ten is read as strong evidence for a diagnosis, but in a Bayesian system fitted to a structured record it is also what a cell of twenty cases produces for any feature common within it and rare elsewhere. We test whether apparent pathognomonicity in scarce diagnoses is a sample-size artifact of predictable size.\nStudy Design and Setting. Two pre-registered analyses on the first-visit record of a university orofacial pain clinic. The substrate is refitted unchanged to 1,501 visits under a fixed penalty, and later by maximum likelihood on a cohort grown to [3,100]. Nine diagnoses holding 16 to 41 cases form a frozen scarce band; high-prevalence diagnoses are comparators.\nResults. The scarce-band median likelihood ratio is projected to fall from [13.9] under maximum likelihood to [7.6] under penalization and to [6.1] on the grown cohort; penalization recovers [81%] of the regression growth produces. [22] of [31] features exceeding ten no longer do. High-prevalence ratios move [3.1] to [3.0].\nConclusion. A small cell and a small category are different objects, and the difference is testable. Many quantitative instruments of modern finance, including debt calendars, life tables, least squares, the normal law, the random walk and rescaled-range analysis, were developed for natural systems that do not respond to forecasts. This work argues that the instruments migrated into finance while their validating condition, a stationary and prediction-indifferent process, did not, and proposes a provenance audit to manage the resulting reflexivity gap. Reproducible tests on public data (622–2026) show fat tails in US returns, a variance-ratio rejection of the random walk that disappears once time-averaging is accounted for, a spurious sunspot–inflation correlation, and long memory in volatility but not in returns. Implications are derived for AI governance in finance. The quantitative phase is a cross-sectional survey (target n=677) using an adapted HIV knowledge questionnaire, the Healthcare Providers' HIV/AIDS Stigma Scale (HPASS), and the Nursing Willingness Questionnaire (NWQ), reported per STROBE guidelines. The qualitative phase involves 15–25 in-depth semi-structured interviews with purposively selected participants, reported per COREQ guidelines, to explain and contextualize the quantitative findings. The study is guided by the Health Stigma and Discrimination Framework, hypothesizing that HIV-related knowledge influences stigma, which in turn influences willingness to treat PLHIV. This review evaluates the comparative diagnostic performance of pancreatic stone protein (PSP) in ICU patients.\nObjective: To evaluate whether PSP demonstrates superior diagnostic accuracy for sepsis compared with CRP and procalcitonin, tested through pooled sensitivity, specificity, and hierarchical summary ROC analysis.\nDesign: A systematic review conducted according to PRISMA 2020 guidelines, using four major databases (PubMed/MEDLINE, Embase, Cochrane Library, Scopus) without language restrictions.\nSetting: The review was conducted at an academic research center specializing in evidence synthesis and critical-care epidemiology.\nMethods: Diagnostic accuracy studies of adult ICU patients reporting PSP, CRP, or procalcitonin were included, with independent data extraction and risk of bias assessment via QUADAS-2. Pooled sensitivity, specificity, DOR, and AUC were calculated using bivariate random-effects and hierarchical SROC models, with subgroup analyses by ICU type, sepsis definition, assay cutoff, study design, and biomarker timing.\nMain Outcome Measure: Pooled diagnostic accuracy of PSP, CRP, and procalcitonin for identifying sepsis in adult critically ill patients.\nSample Size: Fifteen studies were included (n ≈ 3400 patients), consisting of 8 adult ICU studies contributing to the meta-analysis and 4 pediatric studies analyzed narratively.\nResults: PSP demonstrated the highest pooled diagnostic performance, with sensitivity 0.87 (95% CI [0.83–0.91]), specificity 0.82 (95% CI [0.77–0.87]), diagnostic odds ratio 27.5 (95% CI [16.8–44.9]), and AUC 0.91. PCT showed sensitivity 0.82 (95% CI [0.78–0.86]), specificity 0.78 (95% CI [0.72–0.83]), DOR 19.4 (95% CI [11.0–34.2]), and AUC 0.87. CRP demonstrated lower specificity and overall accuracy (AUC 0.81). Heterogeneity was moderate across biomarkers (I²: PSP 49%, PCT 58%, CRP 61%). Subgroup analyses showed PSP maintained superior diagnostic value in surgical and mixed ICUs and with both Sepsis-2 and Sepsis-3 criteria. Sensitivity analyses showed stable results with exclusion of high-risk Emerging evidence suggests that gut-derived microbial metabolites may influence incretin secretion and signaling; however, the specific metabolites involved and the extent of evidence supporting these relationships in humans remain poorly characterized. The purpose of this scoping review is therefore to characterize the current evidence base, identify bacterial metabolites associated with GLP-1 and GIP pathways, describe the populations and methods represented in the literature, and identify important gaps that warrant future investigation.\nA PRISMA-ScR-guided scoping review will be conducted using PubMed, CINAHL, and Scopus. The search strategy will incorporate terms related to the gut-brain axis and gut microbiota, incretin hormones including GLP-1 and GIP, and microbial metabolites and metabolomics. Preliminary searches identified 227 English-language records in PubMed, 57 records in CINAHL, and 50 records in Scopus after application of the respective database search strategies and English-language limits. Titles and abstracts will be independently reviewed by two authors to determine eligibility. Eligible studies will include full-text, peer-reviewed primary research studies conducted in human populations, published in English, and addressing bacterial metabolites relevant to incretin signaling. Additionally, studies involving participants younger than 18 years, participants with conditions that may influence gut microbiome (specifically colorectal cancer, hepatic encephalopathy, celiac disease), samples collected from participants with an active COVID infection or active ulcerative colitis flare up, or samples collected two weeks prior to (or actively) taking antibiotics, antiretroviral therapy or corticosteroids will be excluded. A third author will be consulted to resolve disagreements between reviewers. Citations and screening decisions will be managed using the Rayyan systematic review platform, and agreement between reviewers will be evaluated using Cohen’s kappa statistic.\nData will be extracted using a predefined data extraction form. Extracted information will include study characteristics, publication year, country or state of origin, participant characteristics, microbiota analysis methods, relevant medication or health factors, and primary findings related to gut microbiota and bacterial metabolites. The primary author will conduct the data extraction, with findings confirmed by the secondary author. Study quality will be assessed using established reporting and risk-of-bias frameworks appropriate to study design, including the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) framework for observational studies, the Cochrane Risk of Bias Tool for Randomized Trials Version 2 (RoB 2.0) for randomized trials, and the Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) for nonrandomized studies.\nThe expected outcome is a comprehensive characterization of the current human evidence regarding bacterial metabolites associated with GLP-1 and GIP pathways. A metabolite classification map will be developed to organize identified metabolites according to their microbial source, relationship to incretin signaling, and reported biological effects. Metabolite classes of interest may include short-chain fatty acids such as butyrate, propionate, and acetate; bile acid derivatives; trimethylamine and trimethylamine N-oxide (TMAO); indole and other tryptophan-derived metabolites; and other microbial metabolites identified through the review. The review will also characterize the populations represented in the literature, methodological approaches used to evaluate microbial metabolites and incretin pathways, and the consistency of reported findings across studies.\nThe scoping review is expected to identify areas in which evidence is limited, inconsistent, or absent, particularly regarding human populations and older adults. These findings will establish a clearer understanding of the current state of research on gut bacterial metabolites and incretin signaling and will identify priorities for future investigation. In particular, the findings may provide a scientific foundation for subsequent empirical studies examining whether incretin-related bacterial metabolites are associated with clinical cognitive diagnosis or domain-specific cognitive function in older adults. Such future studies may include investigations of cognitive status, episodic memory, and executive function; however, these analyses are beyond the scope of the current stand-alone scoping review.\nOverall, this scoping review will provide a comprehensive synthesis of the human literature and will establish the current state of human evidence regarding gut bacterial metabolites implicated in GLP-1 and GIP pathways. Eligible studies name at least one of the following endocannabinoid related lipids, anandamide (AEA), 2-arachidonoylglycerol (2-AG, including 1-AG), oleoylethanolamide (OEA) or palmitoylethanolamide (PEA) as a planned quantification target in human CSF. Studies administering agents that alter eCB signalling are included, with pre- and post-administration values extracted separately. PubMed/MEDLINE, Web of Science, Scopus and Embase (Ovid) were searched in January 2026, and the search will be repeated before publication.\nExtracted data cover study populations, sampling and analytical methods, reported CSF concentrations, CSF–blood and within-CSF correlations, and the reporting of potential confounders. Methodological quality will be assessed with JBI checklists and a review-specific eCB method checklist; reporting completeness with items from STROBE-ME, BRISQ and the Lipidomics Standards Initiative checklist. Overlapping cohorts and companion publications will be linked and counted once.
