Corneal surprise during vitreoretinal surgery

A 65-year old man underwent pars plana vitrectomy for dislocated lens fragments into the vitreous cavity during a previous cataract surgery. The right eye anterior chamber exam showed three nylon 10/0 corneal sutures and fundus examination revealed dislocated lens fragments into the vitreous cavity. There was no history of refractive surgery. The patient was scheduled for a standard 20-gauge three-port pars plana vitrectomy. During the surgery a big epithelial bleb appeared in the corneal surface forcing us to stop the procedure. Epithelium removal creating a corneal flap let us a good visibility during the surgical procedure. This easy maneuver allowed vitreoretinal surgeon to finish successfully surgery. Finally, the flap was replaced and a bandage contact lens placed. Corneal bleb was the result of a stitch that perforated the whole corneal thickness. Vitreoretinal surgeons should be alert to the implications of previous anterior segment procedures when performing surgery in these patients.
