Immediate lymphatic reconstruction (ILR) has emerged as a preventative microsurgical technique, though long-term data remain limited and recent studies have raised concerns about durability of benefit beyond 3 years. We evaluated objective and patient-reported outcomes in this high-risk population at extended follow-up.\nMETHODS: We performed a one-time follow-up assessment of patients previously enrolled in our prospective ILR database between 2018 and 2022. The primary outcome was inter-limb volume difference between the affected and unaffected arms. Secondary outcomes included bioimpedance spectroscopy (L-Dex via SOZO), circumference, and validated patient-reported outcome surveys (LLIS, ASES, DASH).\nRESULTS: Of 71 patients contacted, 34 agreed to participate (22 ILR, 12 no-ILR). Mean follow-up was 4.7 years (range 3.6–7.6). The majority of participants were White (64.7%) with 23.5% self-identifying as Black. Mean age was 52.6 years (range 27–72) with BMI 28.3 (range 21.6–43.6). All patients underwent ALND, chemotherapy, and post-operative radiation therapy, with no significant difference in oncologic or reconstructive surgery type between groups. The mean inter-limb volume difference was 2.5-fold smaller in the ILR group compared to no-ILR (124 mL vs. 306 mL); the no-ILR group mean exceeded the established 200 mL diagnostic threshold for lymphedema while the ILR group mean remained well below it. Upper extremity functional scores (ASES) were clinically meaningfully better in the ILR group (90.2 vs. 83.3; difference of 6.9 points exceeding the MCID of 6.4; unaffected arm 99.6). L-Dex bioimpedance and LLIS trended in favor of ILR (6.2 vs. 14.8; 8.1 vs. 15.1), and effect sizes were consistent across all outcomes (Cohen's d 0.43–0.52). The study was underpowered to confirm individual outcomes statistically (n=34); a sample of approximately 126 patients would be required to detect medium-sized effects with 80% power.\nCONCLUSION: At a mean follow-up of 4.7 years, ILR was associated with a 2.5-fold reduction in arm volume differential and clinically meaningful preservation of upper extremity function, with consistent medium effect sizes across all secondary measures. These findings support broader adoption of ILR at the time of ALND in high-risk patients.
